Abstract: Electroencephalography (EEG) provides objective data in human medicine to diagnose epilepsy, measure seizure frequency and evaluate efficacy of anti-seizure drugs (ASDs). Conversely, in veterinary medicine, mainly subjective data, e.g., caregiver-based questionnaires, have been used. Potential concerns with the veterinary approach are: (1) the underreporting of seizures, a known phenomenon in people, and (2) the over-estimation of ASD efficacy. For (1), a retrospective study of (1) found that the coefficient value (rs) comparing reported seizure to EEG recorded seizure frequencies was 0.39 (weak correlation, p = 0.027, 95% confidence interval (CI) = 0.048 – 0.64) in 33 dogs with epilepsy. Subsequently, a follow-up prospective investigation showed that rs is 0.08 (p = 0.84) in 8 dogs with epilepsy. In terms of (2), a preliminary examination of two cases identified worsening of the recorded seizure frequency regardless of the reported seizure frequency. Continued prospective investigation will elucidate these concerns.
